Change of circumstances form

This page explains when you should use the sponsor change of circumstances form, what information you need to complete it and where you should send it.

When to use the sponsor change of circumstances form

You should use this form to notify us of any changes to your details as part of your sponsor duties. This includes:

  • changes to your organisation's details; or
  • changes to the number of certificates of sponsorship needed; or
  • change of authorising officer, key contact or level 1 user; or
  • appointing or removing a representative; or
  • providing organisation takeover details; or
  • providing an update on criminal convictions; or
  • changes to accreditation/registration status; or
  • withdrawing or surrendering an application.

You must fill in the form in block capitals using a black pen. Please enter all dates as dd-mm-yyyy, for example 29-04-2000.

What information you need to complete the sponsor change of circumstances form

You will need the following information to complete the form:

  • details of the changes you want to make;
  • your sponsor licence number or application number;
  • if you are changing the authorising officer, key contact or level 1 user, you will need the following for the new user:
    • name;
    • contact details;
    • email;
    • date of birth;
    • nationality;
    • national insurance number;
    • their immigration status;
    • the home office reference number;
    • the expiry date of their current leave to enter or leave to remain;
    • their passport number;
  • details of any organisational changes including business name change, merger, de-merger or takeover;
  • details of any representatives you want to use, their contact details and their OISC registration number or details of exemption;
  • the number of additional certificates of sponsorship you need or if you are notifying us of your estimated number of certificates of sponsorship needed for the next year of registration;
  • reasons for withdrawing your application for a sponsor licence or surrendering your sponsor licence.
